The proof surface

Why the flat version breaks
A list of phone numbers is not an emergency card. The useful version answers the questions someone would ask in the first ten minutes — who has authority, what allergies matter, which medication is in the house — and marks the ones you have not answered yet.
How to build the card in four passes
Two contacts, medical, medication, who is authorised to collect, and the vet or the equivalent for your household. The card counts them, so a blank one is visible rather than remembered.
Who is allowed to collect a child, who may authorise treatment, who has a key. A number without authority is a phone call someone cannot use.
Where the inhaler lives, which drawer, which vet, where the gas shut-off is, what the gate code is. Those are the answers people need in the first ten minutes, and they are the ones that are never written down.
A card in a drawer that is two years old is worse than a blank one. Set the review date, print or copy the card, and fix the blanks before you put it away.

What breaks first
Leaving required lines blank and calling it done
The card counts the blanks and says so. A partial card reads as complete to anyone picking it up, which is exactly the failure it is designed to prevent.
Using nicknames and unlisted numbers
An emergency card is read by people who do not know your family. Full names, relationships and numbers, written as if a stranger were reading them, because one will be.
Forgetting the reverse direction
The card also has to work when you are the one who is late. Leave a copy with whoever covers for you, and give the school or the childminder the same version.
