Why the flat version breaks
The contract states a notice period in a paragraph and the payment date on a separate line, and nothing joins them. The ledger joins them: freeze or cancel, notice date, last allowed payment, written confirmation, and the date money actually stops leaving.
How to use the membership exit ledger in four passes
Written notice, an email address, a form, an in-person visit, or notice accepted only on the first of the month — the clause decides what counts. Getting this wrong is the most expensive mistake in the whole process because the notice period restarts.
Send it in the form the contract requires and ask for a written acknowledgement with a reference. The date you gave notice is what every subsequent month is counted from, so a phone call with no reference is worth very little.
The gym collects on a fixed day; if the notice period ends after that day, one more payment is due whether or not you attended. Seeing both dates on one line is how people stop paying for months they never used.
Cancelling the direct debit first does not cancel the contract in most terms and can generate a failed-payment fee. Cancel it once the notice period has run and keep the statement that shows no further collection.

Stopping the payment without serving notice usually breaches the contract and can leave a debt passed to a collection agency over a membership you no longer use. Serve notice in the required form, then stop the payment.
Accepting a 'freeze' when you wanted to leave
Freezes often keep the notice clock running or restart it on unfreeze, and some carry a monthly fee. If the goal is to leave, ask for the exit path in writing rather than the generous-sounding alternative.
