Why the flat version breaks

Memory keeps the worst interview and the best one and loses the sequence between them. Six dated scores with the change you intended to make show whether the same stage is failing twice, which is a fixable problem rather than a run of bad luck.

How to use the interview debrief index card in four passes

Score within an hour of finishing

The score is about your performance, not the outcome; an hour later it is still a memory of the room. Use the same five-point scale every time so the numbers are comparable across months.

Write one change, not a summary

'Ask about decision rights' is a change. 'Felt nervous' is a feeling, and feelings cannot be prepared for. If you cannot name one change, the debrief is not finished.

Set your own band floor before the first entry

The floor is the score below which you would change your approach. Setting it in advance stops the flag from moving every time the job market does.

Read the trend and the flagged rows together

A rising line with one recurring flagged stage is a specific problem worth fixing. A flat line with everything flagged means the preparation approach itself needs replacing, not more of it.

Writing the same change five times

Repeating an intention means it was not really a change. If the third entry carries the same note, the fix needs to be different rather than restated.
