Steps
Name the outcome
Write what the project must deliver and who accepts it.
List completed work
Link to the approved files in the system of record.
List open items
Give each item an owner, status, due date, and next action.
Name dependencies
Record decisions, blockers, access needs, and people to contact.
Confirm receipt
Ask the next owner to confirm the files and open items they received.
Worked example
A project lead transfers a draft and two open questions. The handoff names one new owner for each question and links to the approved project folder.
