How to work the observation completeness matrix
Count the slots you intended to complete, not how many symptoms you expected. The plan can be once daily or another agreed cadence.
A recorded no-symptom observation can count as complete. A blank cannot. Use aggregate counts, not diagnoses, names or private diary text.
Missing = planned minus completed. Compare 100 × completed ÷ planned with your chosen threshold; ten of fourteen is about 71.43 percent and falls below the illustrative 80 percent target.
The total counts blanks, including blanks in otherwise above-target rows. Discuss reasons and limitations instead of backfilling from memory as if those entries were observed.
